The CAO will use the following procedures when destroying controlled documents:
Use the PW 771 to maintain a log for each type of controlled document that has been voided and is to be destroyed.
Record the serial number of each document to be destroyed on the PW 771.
Have the Issuing Officer and one other individual designated by the CAO Executive Director witness the destruction of the documents and sign and date the PW 771 attesting to the destruction of the documents recorded on the PW 771.
Maintain the PW 771 in a separate file as a record of documents destroyed.
NOTE: When destroying multi-part controlled documents, make sure to void and destroy all CAO copies of the controlled document.
