In full-service OPTIONS counties, the AAA will not complete the PA 600 for an individual entering an ICF/ID
A face-to-face interview by the CAO is not required for a resident of a State-operated facility. The Facility Reimbursement Officer or designee will interview the individual and/or representative, assist with completion of the PA 600, and obtain the necessary verification and signatures. The Facility Reimbursement Officer or designee may sign the application on behalf of the individual. If the CAO does not conduct a face-to-face interview, the signature of the applicant must be notarized.
The CAO is responsible for the eligibility decision. If information from the facility is unclear, inconsistent, or not verified, the CAO will contact the facility, the individual’s representative, or family members, if necessary, for further information or verification. The CAO will also review resource and income information available on the Income Eligibility Verification
System (IEVS).
The date of application is the date the client or representative signs the PA 600.
