The BCCPT “Creditable Coverage” Verification form can be accessed in Appendix E and saved to the Desktop. Once saved, the form can be accessed, completed and sent as an attachment on the email to the HIPP Regional office associated with the correct County. Step by step instructions are below:
1. Access the BCCPT Template in Appendix E.
2. Click on “FILE”, “SAVE AS.”
3. Select “Document Template” from “SAVE AS TYPE” pull down menu.
4. Click on “DESKTOP”.
5. Type in the “FILE NAME” field “BCCPT Form.dot” and click the “SAVE” button.
6. Close the BCCPT Form.
7. Go to your DESKTOP.
1. Click on “BCCPT FORM.DOT” on your DESKTOP.
2. Complete the “Complete by the CAO” section of the form. (For field help press F1 or check the status bar at the bottom the window.
3. Click on “File”, “Send To”, “Mail Recipient (as Attachment).”
4. This will open MS Outlook with the document attached.
5. In the “To” field type the email address of the HIPP Regional Office for your County (listed below).
6. In the “Subject” field type the “BCCPT (county/record number).”
7. Click on the “SEND” button.
