The CAO must inform service providers that they can get the GLA declaration form from the central program office (OMH, OMR, Department of Aging), or the CAO can give the service provider a copy. (See the sample below--available on Docushare PA-1869.)
CAO management staff must review the service provider's GLA declaration form using the attached chart to examine the extent to which the facility meets criteria. If a facility serves meals, the site provider must meet each criterion of the GLA definition so that residents who are blind or otherwise disabled may be potentially eligible for SNAP.
After determining whether the site meets each GLA criterion, the CAO must notify the provider (and others listed on the form) of its decision using the institution status notice. See the sample below – available on Docushare – PA 1870.
The CAO must compile a list of GLA SNAP institutions in the county. The list must be retained for audit purposes and be made available to all SNAP eligibility workers for use in deciding whether or not an applicant or recipient lives in a GLA.
The CAO must review the status of a facility when residents apply for certification or renewal until a complete list of SNAP GLAs is available. The CAO must also review the facility's status periodically. A resident's application for certification or renewal must be held pending within the 30 day processing time frame until a decision is made as to whether the facility meets the GLA requirements . If the facility meets the requirements, the CAO must put the facility's completed GLA declaration form on file and process the resident's application as per the 30-day processing standard in Chapter 504.42.
Residents found ineligible because the facility fails to meet the GLA requirements must be given a client notice from the PA/FS 162 series. Which form to use depends on whether the decision is being made at the time of application, during the certification period, or at renewal.

The CAO must, in addition to maintaining its own GLA facility status file, supply a copy of the completed GLA declaration form and a copy of the institution status notice to each of the following:
