570.3 Reporting Methods

A change report form (PA 239-SP) is used to report changes. A household may also report by phone, by mail, by fax, electronically, or through someone acting on the household's behalf. The CAO must give the household a local number that accepts collect calls or an 800 number for reporting changes by phone.   7 CFR § 273.12(b)(1) & 7 CFR § 273.12(b)(3)

The CAO must provide the household with a PA 239-SP and a self-addressed postage paid envelope:   7 CFR § 273.12(b)(4)

 

The CAO must keep the PA 239-SP in the case record for three years.
