377.5 Client Verification Request Form

The Verification Request Form (Pa162VR) will be sent when a recipient has not reported information shown on the Income Eligibility Verification System (IEVS) or when information received from dependable sources is different from what the recipient said. The verification form will tell the recipient that he or she has 10 calendar days to respond to the request for information. If the person does not respond, the CAO will confirm the information by contacting someone else.

 

NOTE:  If information was confirmed before through third-party contacts, and if the recipient stays eligible in all other ways, then the CAO will not send a notice. The CAO will process an overpayment, if one occurred. If the CAO gets third-party information that shows the recipient would have stayed eligible or is currently eligible (for example, income from that source ended), the CAO will notify the recipient to reapply by sending him or her an application and a letter of explanation.

 

If the recipient refuses or fails to cooperate within 10 calendar days, the CAO will send an Advance Notice and close the case.
