Appendix J: Procedures for the PA 207

For replacement of three or more checks cashed within a 12-month period, the CAO will complete these steps:

1. Schedule an appointment for the recipient to come to the CAO to sign a PA 207.

2. Complete the appropriate sections of the PA 207 including the investigation block.

3. Have the recipient sign and date the authorization to investigate the cashed check only one time under the certification. The recipient is not required to provide the sample signatures.

4. Send the original signed PA 207 via the U.S. Mail to:

Harrisburg, PA 17105-8001

5. Retain a copy of the PA 207 in the case record.

Treasury receives the PA 207 and:

1. Forwards it to the bank to request that they verify that the individual who cashed the check properly identified himself. If the individual did not provide the proper verification, Treasury asks for repayment of the funds.

2. If the bank responds and indicates they will repay the funds, then Treasury and the Comptroller’s Office determine how the funds should be re-deposited.

3. If the bank does not respond or responds that the PA 207 is not adequate to make the determination, then Treasury reports this to Operations.

Operations process:

1. Operations provides the CAO Executive Director of the respective county/district for the PA 207 of the findings from Treasury.

2. If an overpayment is to be processed, the CAO completes a manual overpayment referral. Once the SSP program is in ARRC/OARS, the referral will be system-generated. The CAO will follow the procedures as outlined in the ARRC/OARS Ops Memo (to be posted).
