The CAO will send the PA 1747 Family Safety Notice and Domestic Violence Verification Form and/or the PA/CS 173GC (“Good Cause” Determination Based on Factors Other Than Domestic Violence for Child Support or Paternity) to the appropriate Area Office (AO). Child Support Enforcement Specialists (CSES) receive the PA 1747 and/or PA/CS 173GC from CAO staff either directly, by email notification that the form(s) have been imaged, or through the Area Office Resource Account.

Email the CSES or the Harrisburg AO Resource account with the electronic copy of the PA 1747 and/or PA/CS 173GC or upload the PA 1747 and/or PA/CS 173GC to eCIS Imaging and email the CSES the case record number informing that the information has been uploaded onto CIS imaging.
