The County Assistance Office (CAO) will record the type of proof used to identify a person.
Acceptable forms of proof may include, but are not limited to, the following:
Social Security card.
Driver's license.
Selective Service card.
Passport.
Military discharge papers.
Voter registration card.
Tribal enrollment or membership documents issued by a federally recognized Indian Tribe
The CAO will accept any document showing the person’s signature, photograph or description as proof of identity.
NOTE: A birth certificate is not acceptable proof of identity.
NOTE: A voter registration card may be accepted as proof of identity by itself only if:
Another form of verification is not possible.
The person has cooperated with attempts to obtain additional verification.
The person has presented consistent information about their situation.
NOTE: The acceptable proof of identity requirements for Medical Assistance (MA) apply when reviewing continued medical eligibility for all Temporary Assistance for Needy Families (TANF) cash closings, including extended medical coverage and changes to eligibility status.
