A change report form (PA 239-SP) is used to report changes. A household may also report by phone, by mail, by fax, electronically, or through someone acting on the household's behalf. The CAO must give the household a local number that accepts collect calls or an 800 number for reporting changes by phone.

The CAO must provide change reporting households with a PA 239-SP and a self-addressed postage paid envelope:

At certification (this is sent automatically by the system to the appropriate households)
At renewal; and
When a household returns a PA 239-SP.
The CAO must keep the PA 239-SP in the case record for three years.
